Request and Share Your Medical Records
You can easily request your medical records and then share them with your pre-authorized healthcare professional or family member. Additionally, when you make requests for copies of your medical records to be sent directly to a healthcare provider for an upcoming appointment, those requests are expedited. As a result, we make every effort to have your records available at the time of your visit.
Parents, caregivers, and legal guardians can easily request and access medical records for a child or a family member, provided they have the proper consent. do not have to pay to obtain copies of medical records. However, there may be instances where patients, healthcare providers, or attorneys will need to pay a fee when requesting for your records. Regardless, you can request medical records by simply completing our records release form and then submitting it by email, mail, or fax.
